3/3/2024 EnchantaVaughn Approved Behavior: Client was confronted by QMHP and asked why he wasnât in a good mood when everyone was being nice to him. Client was silent for over 20 minutes. Client was educated that no one is going to allow him to go anywhere when he has not done anything productive. Client missed several opportunities to make better choices, as he continued to have a meltdown when things didnât go his way. Client was pulled to the side and confronted about his current and displeasing behaviors. Behavior: Client would not own up to anything and continued to look upset. Client finally opened up about what was bothering him. Client expressed feeling let down by his parents, especially his dad and wished they spent more time together. Client is often disappointed by their relationship and it heavily affects him at home with his mom who has to work twice as hard to make him happy. Client is learning to be more respectful to authority and not to take his frustrations out on his mom, who he has a healthy relationship with. Intervention: QMHP facilitated a family discussion to talk about common mistakes children with ADHD make and how he can make better choices. QMHP reviewed home expectations and how client should conduct himself daily. QMHP provided client with a release of energy by dancing to meditative music to help him relax and decrease his hyperactive behavior within the home environment. QMHP showed client how to utilize the fresh air to help gain clarity and empowerment in manifesting what he wants from the universe. Intervention: QMHP processed client feelings and offered choices of what to do when faced with challenges in the home setting. QMHP educated the client with relaxation strategies such as deep breathing and counting to 10 to decrease the clientâs aggressive behavior. QMHP used appropriate behavior modeling by reviewing situations which exhibits positive behavior to decrease the clientâs impulsive behavior and encourage him to use self-control. The client was encouraged to use good judgement, and respect authority figures. Response: The client appeared to be shifting moods with the meditation music. Client utilized relaxation strategies when needed. The client showed his gratitude for being able to release physical tension stating that he feels relaxed and able to catch his breathe again. Client retained a lot of the information he was taught which motivates him to pay more attention and take his time in his therapy. Client is observed problem solving on his own and handling his emotions in more constructive way. Response: Client engaged in self-reflection and reflected on his day, the good and the bad in front of mom and seemed more open minded with the interventions that were displayed. Client practiced some of the examples of alternate behaviors (e.g. closing his eyes and thinking about happy memories, coloring/drawing pictures, repeat positive âI-statementsâ 10-15 times, and ask questions that will help him understand things better) at home when he felt bored or lonely.
